Preserved core rule: positive Biomed FOB plus stool RBC after normalized calprotectin is not an emergency by itself with normal hemoglobin and a high-quality 2024 colonoscopy, but it is not noise. Persistence, iron/Hb drift, visible blood, melena/upper-GI symptoms, or alarm symptoms should move toward GI-source review rather than endless retesting.
